Closing the Care Loop: Addressing Hunger After Hospital Stay

Food Insecurity is a significant barrier to achieving positive health outcomes, particularly for individuals managing chronic conditions or recovering from serious illness. Many patients face uncertainty about where their next meal will come from once they are discharged from the hospital.
To address this need, a program called “Meals to Go” was launched. It provides 4-6 frozen meals to patients who identified as food insecure during their hospital stay. These meals serve as a temporary bridge, offering nourishment and stability until a more permanent solution can be implemented.
