Asthma Under Control: Nurse-Led Quality Initiative in Pediatric Primary Care
Description:
This nurse-led quality initiative in primary care targeted pediatric patients needing asthma education and updated Asthma Action Plans. Using gap lists and outreach, families were scheduled for RN-led visits that provided hands-on inhaler training, tailored education, and seasonal preparation. Inhaler education visits increased from 9 to 16 per month, duplicate inhalers were corrected, and prescription adjustments saved families over $2,000 annually. Outcomes included improved symptom control, reduced emergency reliance, stronger patient–nurse relationships, and greater nurse confidence in delivering education and digital care.
Learning Objectives:
Describe how a nurse-led quality initiative in primary care improves pediatric asthma outcomes through proactive identification, inhaler education, and updated Asthma Action Plans.
Explain the impact of targeted caregiver engagement and tailored education on symptom control, emergency department utilization, and family-centered care.
Evaluate the effectiveness of nurse-led interventions in reducing medication errors, lowering family financial burden, and enhancing nurse confidence in patient education and digital tools.
