From Transition to Trust: Strengthening Handoffs After Fetal Loss

Several Patient Safety Reports (PSRs) revealed instances where products of conception (POC) - tissue from a pregnancy, including the embryo/fetus, placenta, and membranes were at risk of being lost or discarded before being transported for cremation or parents could retrieve them. This project sought to address this deeply sensitive gap in care by examining the management of products of conception.
