Access Without Borders: Virtual Paneling in Modern Healthcare Delivery
Description:
Access data and patient feedback demonstrates patients’ engagement with novel care delivery models. The development of a virtual primary care department represents a strategic initiative to expand patient access and transform the way care is delivered. This model is designed to establish longitudinal relationships between patients and their designated primary care physicians, ensuring continuity, coordination, and a trusted source of care over time. By creating the infrastructure and staffing necessary to support this approach, the department offers patients a flexible scheduling option that is available 24 hours a day, seven days a week, year-round. This innovation addresses evolving patient needs by removing traditional barriers to access, enhancing convenience, and delivering patient-centered primary care in a virtual environment.
Learning Objectives:
After attending this Abstract, the attendees will be able to explain key factors of the staffing and infrastructure that make up a successful virtual patient empanelment program including patient eligibility criteria, virtual care team model, and referral workflows for onsite care.
After attending this Abstract, the attendees will be able to distinguish considerations and barriers to implementing virtual care models, and apply lessons learned to their own virtual care projects.
