From Review to Results: Connecting the Frontline through Takeaways

Dedicated event reviews offer a valuable opportunity to identify system vulnerabilities and improve patient outcomes. Although our event review participants were consistently recognized for conducting thorough and effective event reviews, we identified a significant gap: the insights and lessons learned were not being consistently communicated to frontline teammates through a standardized process. To close this gap, we developed a structured workflow for capturing and disseminating key takeaways, focusing on safety expectations, communication breakdowns, and workflow inefficiencies. These insights were integrated into standard weekly huddle topics and shared proactively with frontline staff. The goal of this initiative is to ensure that every event becomes a learning opportunity to prevent future harm. Early results demonstrated improved staff awareness, reduced repeat safety events, and enhanced interdisciplinary collaboration. This scalable model supports a culture of safety and continuous learning, offering a replicable framework for other institutions seeking to strengthen patient care through actionable reflection.
